SCHOOL EYE EXAMINATION INFORMATION
Illinois, Public Act 95-671 requires that kindergarteners and all students enrolling for the first time in a public, private or parochial school must have an eye examination. The requirement went into effect January 1, 2008.
Exams must be completed by a licensed opthalmologist or licensed optometrist. This form must be submitted to the school by October 15th of the current school year. If the student fails to present proof of exam by October 15 of the current school year, the school may hold the student’s report card.
