The State of Illinois requires that all Kindergarteners submit a copy of their birth certificate and a Physical (dated within one year of the first day of school) prior to the first day of school. Dental Examinations must be submitted by May 15th and dated within 18 months of the deadline. Vision Examination are due to the office by October 15th.
